		<description>I always find it best to make friends with someone in the office.  That way when you need an appointment in a hurry or a paper signed they may advocate for you.  It stinks but sometimes it seems that&#039;s the way its done.

I&#039;ve been in similar situations with forms. (Docs here don&#039;t give out their email addresses and I&#039;ve heard of patients being penalized for using them if they somehow find them.)
What I&#039;ve had to do is make an appointment to get the form signed.  
Yes, it is annoying and draining but unfortunately it seems to be the most efficient way to accomplish anything. Even getting a new prescription written based on formulary changes.
Even with an appointment just for paperwork it may not help. (We never did manage to get some student loan papers filled out &#039;right&#039; before the GP got annoyed.) However, this has been what has worked best for me.
I will add that I&#039;m lucky and can usually get an appointment with my GP in less than two-three weeks. If your GP&#039;s office works differently then this might not work.</description>
